The following insurances are accepted by Benhaven Counseling-Medical Mutual all products. Anthem Blue Cross Blue Shield. United Healthcare. Value Options (Beacon) Aetna. Cleveland Clinic (Tier 1) Cigna all products. EAP (Please note this is a partial. $250 copay, and plan pays 100% ^ $250 copay, and plan pays 100% ^ Urgent Care Facility. Includes Professional, X-ray and/or Lab services performed at the Urgent Care Facility and billed by the facility as part of the urgent care visit. An additional in-network per scan copay of $65 applies to Advanced Radiological Imaging. Copay — The set fee you pay at every therapy session, after your deductible is met. A copay typically ranges from $15 to $50 per session. B) In-network vs. Out-of-network In-network: If your therapist is in-network, you will have to pay a copay and/or deductible (see section A).
EHP plan members have the benefit of utilizing both the EHP contracted provider network and the Cigna PPO contracted network. To locate a health care or durable medical equipment provider, start by searching the EHP network.
EHP Network
The EHP network gives you access to 14,000 health care providers and 30 hospitals in Maryland, ensuring that you can find care and services near you. The EHP Network includes primary care, specialist, behavioral health, and other providers, as well as urgent care, hospitals, and other medical facilities. Search the EHP network
Cigna PPO Network
The Cigna PPO network is a supplemental provider network, giving you access to more than one million providers and hospitals nationwide, including Maryland. EHP covers all health care services received from providers in the Cigna PPO network at the in-network benefit level and you may use a Cigna PPO network provider as your primary care provider.
Using the Cigna PPO provider search feature: enter the city, state and/or zip code for where you are trying to find care. Then, click either 'Doctor by Type,' 'Doctor by Name,' or 'Health Facilities' to search for the type of provider you want. You will be able to click 'continue as guest' to complete your provider search or create a MyCigna account (now or at later time) to login for additional features.

Cigna Copay For Therapy
No appointment needed, Johns Hopkins OnDemand Virtual Care has providers ready to care for you 24/7. In no time at all, a health care provider is available to review symptoms and prescribe medications, as necessary.
Johns Hopkins OnDemand Virtual Care is available to EHP members and dependents of Johns Hopkins Health System Corporation (including The John Hopkins Hospital, Bayview Medical Center, Howard County General Hospital, Suburban Hospital and Sibley Memorial Hospital), Johns Hopkins University, Broadway Services and Student Health Plan. This service is for adults and children age 3 and older. Anyone under the age of 18 must be accompanied by an adult.
Do not use Johns Hopkins OnDemand Virtual Care for emergency medical matters.

Pre-Authorization Requirements
All EHP plans require pre-authorization prior to any hospital stay, but coverage and copays for other medical services vary among plans. EHP providers should work with you and EHP on pre-authorization requirements for all inpatient hospital stays and any outpatient procedures that require pre-authorization. Be sure to review your Summary Plan Description (located at your HR office) to see your medical benefits.
Out of Network
If you need to treat a non-urgent condition and you cannot locate an in-network EHP or Cigna PPO provider, you may still be covered. Your plan may offer an out-of-network option for many services. To determine whether specific urgent and non-urgent services are covered out-of-network, or to determine any copay or reimbursement responsibilities, review your Summary Plan Description, which is available through your HR office, or visit the Benefits Explorer.
Find a Lab
Use the Provider Search Directory tool to find a lab. In step 3, under 'I would like to see…' check the Radiology and Lab option.
